About Pennsylvania Workers’ Compensation Forms

After suffering a work-related injury, an injured employee must meet mandatory requirements to be deemed eligible for workers’ compensation benefits. Included in these requirements is the use of certain forms for providing information such as medical reports, as well as applications, authorizations, and agreements for various actions and events.

A First Report of Injury (FROI) electronic data interchange or Web portal transaction is required to establish a claim in the Workers’ Compensation Automation and Integration System, also known as WCAIS. Any forms received by the Bureau of Workers’ Compensation (BWB) prior to submission of this mandatory initial FROI transaction cannot be uploaded into WCAIS since no claim has already been established in the system to which the form can be filed or attached.

For certain actions, submission of the EDI transactions alone will not satisfy the filing requirements under the Pennsylvania Workers’ Compensation Act. Thus, for these actions, there are requirements that certain forms be submitted to the BWB. These include compensation and settlement agreements for disability, permanent injury or death.

In addition to the EDI transaction, certain forms must be sent to the injured worker and filed with the bureau. These include wage statements and other notices regarding benefits and whether they are payable or denied. Written notice to the injured worker has not changed and is still required by Pennsylvania law.
